Back To School Documents
This year our back to school documents can be completed either online or hardcopy. All of our back to school documents are listed below.
You may open the documents, fill out electronically, and email them to Nettie Alexander our Media Specialist at firstname.lastname@example.org. Each student has a Google account so they may be completed within that account and emailed. When you open the documents it will ask for you to make a copy. Make a copy. Fill Out. Email back.
each student will receive a hard copy, fill out the hard copy, and return to your child's teacher.
Either way you fill them out, please do so ASAP so that we have them on file. The documents that MUST be returned are: 2. Attendance Letter, 4. Health History Form, 6. Behavior Contract, 7. Occupational Parent Form, 8. School Parent Compact Letter, and 9. Signature Form
